Go365 for Humana Healthy Horizons® is a well-being and rewards program that gives you the opportunity to earn rewards for completing healthy activities, including getting health screenings and working with a Wellness Coach.
For each eligible Go365 activity completed, you earn rewards that can be redeemed for e-gift cards in the Go365 in-app mall. Rewards earned through Go365 have no cash value and must be earned and redeemed in the same plan year. Go365 for Humana Healthy Horizons is available to all members who meet the requirements of the program.

*For screenings, you may experience a delay in rewards showing up in your account. Please note that these rewards are dependent on your provider submitting claims in a timely fashion and can take up to 90 days to process.
Go365 for Humana Healthy Horizons is available to all who meet the requirements of the program. Rewards are not used to direct you to select a certain provider.
Gift cards cannot be used to purchase prescription drugs or medical services that are covered by Medicare, Medicaid, or other federal healthcare programs; gambling, alcohol; tobacco; e-cigarettes; or firearms. Gift cards must not be converted to cash. Rewards may be limited to once per year, per activity.
